HIPAA Policy

In order to share your medical information with Attilio, Junction needs your permission. By signing below, you are authorizing Junction to share your medical information (such as medical records, lab orders, and lab test results) with Attilio One Inc. (“Attilio”).

Purpose. Your information will be shared so Attilio can review your test results for the purpose of managing your care.

Your choice. You are not required to sign this authorization. But if you do not sign, you won’t be able to use laboratory testing services provided through Junction.

Right to revoke. You may withdraw this authorization at any time by emailing info@attilio.com. It may take a few business days for Junction to process your request.

No effect on other care. If you choose not to sign this authorization, it won’t affect other aspects of your care. So, for example, your healthcare providers will not refuse treatment, payment, health plan enrollment, or benefits.

Possible re-disclosure. Once your information is shared with Attilio, it may no longer be protected by federal medical privacy laws and could be shared again by Attilio.

Overrides prior restrictions. This authorization overrides any instructions you may have given your healthcare providers in the past limiting disclosure of this information to Attilio.

Duration. This authorization will remain in effect until you revoke it in writing or until Junction’s agreement with Attilio ends.

Copy. You are entitled to receive a copy of this authorization.

By signing below, you confirm that you have read and understand this authorization and agree to the sharing of your medical information as described above.
